To ensure your safety and meet legal requirements, we ask that you complete this consent form on the same day as your appointment.

If you’reunder 25, you may be asked to show valid photo ID — don’t be offended; it’s for your safety and legal compliance.All clients must bringoriginal photo ID. We do not accept photocopies or digital images of identification. Appointments cannot proceed without a completed and signed form.
⚠️ Health Information Note
If you answer “yes” to any medical questions, we may require a doctor’s note before proceeding with your tattoo. This ensures your safety and allows our artists to give you the highest standard of care.
