Please complete this consent form on the day of your appointment. It helps us provide a safe and professional experience.
🔞 Age-Based Piercing Guidelines
• 10–12 years: Ear lobes only. Parent/legal guardian must attend and co-sign.
• 13–16 years: Ear lobes and nostrils. Parent/legal guardian must co-sign.
• 16+ years: All piercings except nipples.
• 18+ years: All piercings.
🪪 ID & Guardian Documentation
• Clients under 25 must show original photo ID.
• Under 16s must bring photo ID and either:
• ID with the same surname as guardian, or
• Additional documentation confirming legal guardianship (e.g. birth certificate).
• No photocopies or digital images accepted.
Appointments cannot proceed without valid ID and a completed form.
